Probably a lift/leveling block, and the first time he hit the brakes with that extra trailer weight and that extra leverage peeled the axle from the u bolts. Or they were installed incorrectly. That's my internet guess.
Yeah, I noticed that too. It could have happened at a slow speed. Like crawling in traffic, and then a sudden jab at the brakes. I'd think if it was a higher speed situation, that axle would have left the building, and the truck and trailer would not be on the road still in the lane.
I can just see Mr. Mayhem laughing and saying "And if you have cut rate insurance you might be paying for this yourself..."
