Didn't someone make a tech form I could edit?

If I remember right someone had a Tech form that would allow you to edit the form online then print. Does anyone have this form still?? If so could I get it?? Would like to just print them out.

Second I retired, but I am making a Rodger Clemons type comeback steriods and all, Guess I never thought about how that turned out.

The tabbnig isn't correct at the moment, but you can enter in all the data and print out however many copies you need.
