How long? If more than a few days dont stay in Key West the whole time. Stay a couple days there and then go to Marathon, or the upper keys for a few days. PR is nice . Old San Juan, go snorkeling , see the Jungle, etc. Nice since its US no customs coming home.
Personally, we like going to resorts in Cancun. We usually do that 3-4x a year, and will stay 6 days. There are loads of different shows, restaurants, and activities at the resorts, and they all have nice gyms. Then you can do stuff outside like swim with dolphins and shit.
Just go to St. John, make life simple, key west is a shit show, Puerto Rico is cool, Culebra/ Vieques, Rincon is a cool drive from San Juan, , but need to speak at least Spanglish to have a good time not staying in a resort
Not sure what you are looking for, the D.R. is an option for extreme cheap decent resorts on the beach.

Just got back from Duck Key last week. Very pretty, very laid back. The keys are beautiful. Key West is cool for a day or two, but I wouldn't stay much longer. Eat and drink at Blue Heaven. Good food. Gooder drinks. Other than that, we stayed on Duck Key aside from dinner and ice cream runs.
RE: PR If you rent a car there, prepare for driving in PR by entering a figure 8 demolition derby this weekend. Then you will feel comfortable driving in San Juan. The rental car you receive will look worse than any clapped out SV650 in the entire history of the beeb.
