Hey, it was seen near Raleigh, NC by a buddy of mine. The state that bought us Carolina Squat. My guess is maybe it's for practical reasons, like they haul really heavy stuff in it. Don't know if they beefed up the suspension as well.
I did a quick Google search and found this: Well, a dually pickup is pretty common — but Ford didn't actually make any dually vans. This one was converted to its current state, presumably using pickup parts. Dually-converted vans are pretty rare, but they're not unheard of.

Tow vehicle. There used to be a few dually Suburbans around my area pulling dirt cars. I'm not going to Google if GM ever built them, but I know the one guy owned a body shop and put his own together.
